1. Enquiry/Referral
When an enquiry/referral is received, a member of staff will contact the person to find out any further information required about the person they are looking to place with us within 24 hours.
2. Assessment
Before we can start providing support for the person, we will carry out an assessment to understand their needs, abilities and personality to ascertain if our service is suitable for them.
3. Transition
Following the assessment, a specifically tailored transition plan is put in place. The plan will outline how we will work with everyone involved to ensure that the individual’s transition into their new home is as smooth as possible.
4. Support starts
We begin providing the support the individual requires.
